Enjoy a sign-on bonus of $500, distributed as follows: $250 upon completing the first week of training and an additional $250 after successfully finishing the first 90 days.
Role Details:
Title: Case Management Nurse – Part-Time
Compensation Rate: Starting at $34.00 per hour
Weekly Hours: 20 hours
Schedule: Monday to Friday; Day shift with a flexible timetable
Key Responsibilities:
- Conduct 1-2 home visits daily.
- Perform in-person meetings and home assessments.
- Execute clinical eligibility evaluations for adult foster care participants.
- Process prior authorizations.
- Work autonomously with limited oversight.
- Ensure compliance with programmatic and contractual standards.
- Support program organization, expansion, and staff training for program implementation in various areas.
Qualifications:
- Must possess a valid MA RN license.
- Preferred: Minimum of 2 years in human services.
- Valid driver's license, good driving record, and reliable transportation required.
- Understanding and respect for diverse populations.
- Strong communication, interpersonal, teamwork, and listening abilities.
- Proven reliability and dependability.
