Job Summary

The Gandara Center is seeking a skilled Case Manager to join our team. As a Case Manager, you will provide advocacy and support services to clients, develop strong relationships, and ensure client care and safety. This is a challenging and rewarding role that requires strong communication and interpersonal skills, as well as the ability to work in a fast-paced environment.

Key Responsibilities
  • Provide advocacy and case management services to clients
  • Develop strong, supportive relationships with clients and provide for client care and safety
  • Complete all clinical documentation and casework for assigned residents
  • Maintain updated clinical notes and reporting in accordance with established record keeping procedures
  • Facilitate daily evidence-based psychoeducation groups and other recovery-based groups
  • Provide weekly individual case management sessions with assigned residents
  • Administer and complete the EIM/ESM forms and adhere to funding sources requirements
  • Maintain direct supportive contact with residents to continually assess their individual needs, provide information, discuss problems, set limits, and encourage growth and development
  • Develop and maintain a working relationship and knowledge of other social service agencies in the community
  • Notify supervisory staff immediately in the event of a client crisis, suspected illegal activity, or any behavior constituting an incident in the program
  • Administer drug/alcohol screens to residents when considered clinically appropriate
  • Provide orientation of services to prospective clients, family members, and community interest groups
  • Utilize de-escalation techniques to reduce, prevent, or resolve escalated resident conflicts
  • Acquire and maintain current the necessary certifications mandated by the Agency and funding sources

Requirements
  • Associates Degree + 2 years experience
  • Bachelor's degree + 1-2 years experience preferred
  • Demonstrated strong communication and interpersonal skills, including conflict resolution
  • Experience providing substance abuse services
  • Demonstrated organizational skill, able to multi-task
  • Demonstrated ability to lead through example
  • Must have valid driver's license and meet Agency requirements for driving history records
  • Must have availability of an automobile to transport clients or perform other supportive services as necessary
