Job Summary

We are seeking a highly skilled and detail-oriented Continuum Care Coordinator to join our team at Kaiser Permanente. As a key member of our healthcare operations team, you will play a critical role in supporting the daily activities of our case managers and physicians, ensuring data integrity, and coordinating data input, analysis, and reporting.

Key Responsibilities
  • Support Non-Clinical Functions: Provide administrative support to our case managers and physicians, ensuring seamless coordination of care.
  • Facilitate Discharge Planning: Coordinate discharge planning for members across the continuum, ensuring timely and efficient transitions of care.
  • Answer Inquiries: Respond to patient, vendor, and healthcare recipient inquiries in a timely and professional manner.
  • Enter Authorization/Referral Data: Accurately enter authorization and referral data into required applications, ensuring correct admitting, principal, and discharge diagnoses, bed types, discharge date, and other applicable fields.
  • Ensure Data Integrity: Review and update authorizations daily, and attend case management rounds to update diagnoses or other information as needed.
  • Document Outside Medical Services: Document outside medical services and enter referrals into Tapestry for all applicable levels of care.
  • Ascertain Eligibility and Benefits: Verify eligibility and benefits for all admissions and every 7 days thereafter (or other designated time frame).
  • Coordinate Data and Reporting Needs: Provide timely utilization data and analytic support, and collaborate with other organizational units and outside facilities to obtain, verify, and/or provide information.
Requirements
  • Experience: Minimum two (2) years of relevant experience in healthcare operations or a related field.
  • Education: High School Diploma or General Education Development (GED) required.
  • Certifications: Certified Professional Coder, Certified Outpatient Coder, or Certified Coding Specialist within 6 months of hire.
  • Skills: Strong analytical and communication skills, expertise with tracking, spreadsheets, and graphic computer systems, knowledge of health plan benefits, and understanding of medical center operations and complex health systems.
Preferred Qualifications
  • Clinical Background: Previous experience in a clinical setting preferred.
  • Medical Terminology Certification: Medical terminology certification preferred.
  • Bachelor's Degree: Bachelor's degree preferred.
