Private Duty Nurse

Aveanna Healthcare is seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team of dedicated healthcare professionals. As a Private Duty Nurse, you will play a vital role in providing high-quality patient care and support to our clients in the comfort of their own homes.
Key Responsibilities- Deliver Quality Patient Care: Provide skilled nursing care to patients in their homes, under the supervision of a Registered Nurse, in accordance with the developed care plan and physician orders.
- Assess and Document Patient Needs: Continuously observe and assess patient condition and care needs, documenting all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
- Implement and Update Care Plans: Participate in the development, implementation, and update of individualized care plans to meet the unique needs of each patient.
- Collaborate with Healthcare Team: Work closely with the healthcare team, including physicians, therapists, and other healthcare professionals, to ensure comprehensive and coordinated care.
- Provide Education and Support: Educate patients and their families on disease management, self-care techniques, and prevention strategies to promote optimal health outcomes.
- Education and Licensure: Graduate of an accredited school of nursing and current, unrestricted state license as a Licensed Nurse in the state of practice.
- Certifications and Experience: Current CPR certification and at least six months of recent experience as a Licensed Nurse in a clinical care setting, preferably in home health or pediatrics.
- Skills and Abilities: Strong communication and interpersonal skills, with the ability to work independently and as part of a team, and to prioritize tasks and manage time effectively.
