Job SummaryWe are seeking a highly skilled and compassionate Primary Care Physician to join our team at ChenMed. As a Primary Care Physician, you will be responsible for providing high-quality, patient-centered care to our seniors, working closely with our care teams to achieve exceptional health outcomes.
Key Responsibilities- Provide comprehensive primary care to patients with acute and chronic illnesses, including geriatric patients.
- Develop and implement individualized care plans to improve patient outcomes and quality of life.
- Collaborate with our care teams, including care promoters, care facilitators, and care coordinators, to ensure seamless patient care.
- Participate in quality improvement initiatives and contribute to the development of best practices in primary care.
- Stay up-to-date with the latest evidence-based medicine and clinical guidelines to provide the highest level of care.
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
- Current State Medical License or ability to obtain one.
- Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross (ARC).
- Ability to work independently and collaboratively as part of a team.
