Care Coordination Opportunities
Samaritan Health System is seeking a dedicated Registered Nurse to join our Home Care team as a care coordinator. This is an excellent opportunity for nurses who want to make a meaningful difference in the lives of patients and their families.

This full-time position offers a salary range of $70,000 - $80,000 per year, including a $5,000 sign-on bonus with a 2-year work commitment. In addition to a competitive salary, you will also receive gas mileage reimbursement at the federal rate and a comprehensive benefits package.

The successful candidate will be responsible for coordinating patient care in their place of residence, working closely with physicians and other healthcare professionals to develop and implement individualized care plans. They will also be responsible for providing direct supervision to LPNs and HHAs, as well as providing on-call services as required.

The minimum education requirement for this role is a graduate degree from an accredited school of nursing, and candidates must hold current NYS RN licensure. Salary Range: $70,000 - $80,000 per year
