Job Summary:

The Registered Nurse will plan, organize, and direct home care services, utilizing their expertise in nursing and community health education. The ideal candidate will possess excellent observation, verbal, and written communication skills, as well as problem-solving skills and basic math skills.

Responsibilities:
  1. Conduct initial assessments of patients and families to determine home care needs, providing a complete physical assessment and history of current and previous illnesses.
  2. Regularly re-evaluate patient nursing needs and initiate necessary revisions to the plan of care.
  3. Develop and implement care plans that establish goals based on nursing diagnoses and incorporate therapeutic, preventative, and rehabilitative nursing actions.
  4. Administer medications and treatments as prescribed by physicians, provide direct patient care, and counsel patients and families on meeting nursing and related needs.
  5. Identify discharge planning needs and implement prior to discharge of patients.
  6. Act as Case Manager when assigned by Clinical Supervisor, coordinating patient care for assigned caseloads.
  7. Prepare clinical notes and update primary physicians as necessary, communicating with community health-related persons to coordinate care plans.
  8. Participate in on-call duties as defined by the on-call policy and ensure arrangements for equipment and other necessary items and services are available.
Requirements:
  • Graduate of an accredited school of nursing with one to two years of recent acute care experience in an institutional setting, home health care experience preferred.
  • Current licensure in the state and CPR certification.
  • Management experience not required, but responsible for supervising LPNs and home health aides.
  • Prolonged or considerable walking or standing, able to lift, position, or transfer patients, and lift supplies and equipment.
  • Visual acuity and hearing to perform required nursing skills.
  • Licensed driver with an automobile in good working order and insured in accordance with the organization's requirements.
