About the Role:

We are seeking an experienced and compassionate Licensed Practical Nurse (LPN) to join our team as a Care Coordinator. The successful candidate will be responsible for evaluating the skills competency of our paraprofessionals, conducting annual performance evaluations, and providing supervision visits in the field.

Key Responsibilities:

  • Evaluate and assess the skills competency of our paraprofessionals on an ongoing basis.
  • Conduct annual performance evaluations to ensure our paraprofessionals are meeting the highest standards of care.
  • Provide supervision visits to our paraprofessionals in the field to ensure they are providing high-quality care.
  • Assist our Registered Professional Nurses with paraprofessional in-service education, tracking physician orders, and reviewing progress notes.
  • Collaborate with our Supervisory Nurses to ensure compliance with all regulatory requirements.

Requirements:

To be successful in this role, you will need to be a currently licensed LPN in the state of NY, with at least 1 year of experience in a similar role. You will also need to possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
