Key Responsibilities- Person-Centered Planning: Develop and implement individualized care plans, working closely with clients, families, and healthcare professionals to ensure seamless coordination of services.
- Case Management: Coordinate and facilitate access to community resources, services, and referrals, ensuring timely and effective support for clients.
- Advocacy and Support: Provide emotional support, guidance, and advocacy for clients and their families, empowering them to make informed decisions and navigate complex systems.
- Documentation and Reporting: Maintain accurate and comprehensive records, documenting client progress, and submitting reports as required.
- Team Collaboration: Work collaboratively with multidisciplinary teams, including healthcare professionals, social workers, and other support staff, to ensure comprehensive care and support.
- Professional Development: Participate in ongoing training and education, staying up-to-date with best practices, and expanding your knowledge and skills to provide exceptional care.
- Education: Bachelor's degree in a Human Services field or related field, or equivalent experience.
- Experience: Minimum 2 years of experience in a mental health or social services setting, with a focus on case management and support services.
- Skills: Excellent communication, documentation, and interpersonal skills; ability to work independently and as part of a team; strong problem-solving and critical thinking skills.
- Certifications/Licenses: Valid NC driver's license and personal vehicle insurance coverage; current license or certification in a relevant field (if applicable).
The Intensive In-Home Qualified Professional/Case Manager will work primarily in clients' homes and communities, with occasional travel to meetings and training sessions. A valid driver's license and reliable transportation are required.
