Optum is seeking a talented and dedicated Nurse Practitioner to join our Home-based Medical Care team in the St. Louis, MO area.

In this role, you will have the opportunity to work closely with patients and their families to develop and implement personalized care plans that meet their unique needs and goals.

The ideal candidate will have experience in primary care, home-based medical care, geriatrics or palliative care, with a strong understanding of long-term care, community resources and cost-effective alternatives regarding patient care delivery systems.

With a salary of approximately $120,000 per year, this position offers a rewarding opportunity to advance your career as an advanced practitioner while making a positive impact on the lives of patients.

Key Responsibilities:

  • Conducts initial comprehensive assessments on newly enrolled patients and provides ongoing care thereafter
  • Develops and carries out patients' goals of care in all phases of the patient journey
  • Collaborates with healthcare teams to ensure seamless care coordination
  • Stays up-to-date with the latest clinical standards and guidelines

Requirements:

  • Completion of Nurse Practitioner accredited program
  • Certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC)
  • Current, unrestricted RN licensure in State of Practice
  • Current, unrestricted NP licensure in State of Practice
