POSITION OVERVIEW AND RESPONSIBILITIES

The Healthcare Service Coordinator - Licensed Vocational Nurse (SC-LVN) plays a crucial role in collaborating with medical, behavioral, and social support professionals within the Service Coordination team. This position is dedicated to conducting a comprehensive assessment of STAR+PLUS Members' unique dynamics, requirements, and preferences. The SC-LVN is responsible for delivering educational resources and health-related information to the Member, their Representative, and their Support Network.

In this role, you will coordinate both covered and uncovered Long Term Services and Supports (LTSS) for STAR+PLUS Members, ensuring that their physical, behavioral, functional, and psychosocial needs are effectively met. Adherence to Community First Health Plans, Inc. (Community First) policies, procedures, and protocols is essential for fostering positive working relationships with colleagues, Members, and visitors.

EDUCATIONAL BACKGROUND AND EXPERIENCE

A minimum of three years of clinical nursing experience is preferred. Additionally, at least two years of experience in a managed care setting or working with individuals with disabilities and vulnerable populations facing chronic or complex health conditions is desirable. Proficiency in data entry is required, and call center experience is preferred. Familiarity with RUG/MDS/PASRR is advantageous.

LICENSURE REQUIREMENTS

A current, unrestricted license as a Licensed Vocational Nurse in the State of Texas is mandatory.
