About the Role

The Patient Care Manager is responsible for the supervision and coordination of clinical services, ensuring that patients receive the highest level of care in the comfort of their own homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Key Responsibilities
  • Supervise and coordinate clinical services to ensure high-quality patient care
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Conduct regular patient assessments to ensure ongoing needs are met
  • Lead and mentor clinical staff to ensure they have the necessary skills and knowledge to provide exceptional patient care
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
Requirements
  • Current RN licensure in the state of practice
  • Current CPR certification
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
