Job Summary:

The Disease Management Specialist provides comprehensive healthcare services to individuals and families, focusing on infectious diseases such as HIV/AIDS, Hepatitis C, and STIs. This role requires a high level of empathy, compassion, and respect for patients from diverse backgrounds.

Key Responsibilities:

  • Providing primary care services, including diagnosis, treatment, and consultations
  • Prescribing medications within the scope of practice
  • Teaching patients about preventive care, medical issues, and medication use
  • Offering HIV pre-exposure prophylaxis (PrEP) and providing primary care and specialty care for patients living with HIV infection/AIDS
  • Performing assessments, diagnoses, and treatments for STIs, including screenings and immunizations

Requirements:

  • A minimum of 3 years of clinical experience as a Nurse Practitioner or Physician Assistant in primary care, with an emphasis on STI and HIV
  • Ohio License (NP/PA) and DEA required
  • Experience treating patients with Hepatitis C, Hepatitis B, and HIV/AIDS is essential
  • Demonstrated knowledge and ability to work with homeless, LGBT, multicultural, and diverse populations

Compensation:

We offer a competitive salary range of $120,000 - $150,000 per annum, depending on experience.
