Job Description

Job Summary:

We are seeking a skilled and compassionate RN Care Manager to join our team at Ohio's Hospice. As a RN Care Manager, you will play a vital role in providing high-quality care and support to patients and their families at the end of life.

Key Responsibilities:

  • Assess patient needs and develop individualized care plans to ensure optimal care and comfort.
  • Collaborate with interdisciplinary teams to coordinate care and services.
  • Provide education and support to patients and families on end-of-life care and hospice services.
  • Conduct regular patient assessments and updates to ensure accurate and timely documentation.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.

Requirements:

  • Active Registered Nurse license in the State of Ohio without any board actions.
  • Two years acute care nursing experience preferred.
  • Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment.
  • CPR Certified.
  • Computer skills sufficient to properly document services and care.
  • Ability to drive during daytime, nighttime or inclement weather.
  • Valid Driver's License with Safe Driving Record.
  • State Minimum Automobile Insurance Coverage.
