Job Summary:
The RN – Case Manager is responsible for delivering skilled nursing care and case management for patients in their homes. This role involves admitting patients, coordinating services, and ensuring continuity of care. The RN will work collaboratively with an interdisciplinary team to enhance patient outcomes and provide quality care.

Key Responsibilities:
  • Deliver skilled nursing care and coordinate services for patients and their families in a home setting.
  • Conduct assessments, evaluations, and plan interventions based on patients' physical and psychosocial needs.
  • Supervise Home Health Aides and CNAs, ensuring compliance with care plans.
  • Facilitate effective communication and case conferences with other healthcare professionals and community agencies.
  • Maintain accurate patient records in accordance with agency standards and regulatory requirements.
  • Participate in team scheduling and on-call rotations to meet patient care needs.
  • Engage in quality improvement initiatives and attend training sessions as required.
  • Perform additional duties as assigned by the supervisor.

Qualifications:
Applicants must possess a current RN license in the State of Montana (BSN preferred), a valid driver's license, and reliable transportation. A minimum of 2 years of recent experience in acute care, home care, or hospice is preferred, along with proficiency in clinical skills such as venipuncture and wound care.
