Provide direct support services to individuals residing in the facility. Assist the Residence Manager with the daily operations of the program.

Key Responsibilities:
+ Execute direct care tasks as assigned.

This includes, but is not limited to:
+ Promoting independence in all aspects of the residents' lives while treating them with dignity and respect. + Managing housekeeping, meal preparation, recreational activities, and addressing dietary needs. + Following the residents' Individual Program Plans (IPP's). + Supervising all assigned Direct Support Professionals, which includes instructing, modeling, and observing staff to ensure compliance with assigned tasks and adherence to regulations/policies, as well as maintaining up-to-date assignment sheets. + Coordinating shifts; ensuring that active programming is completed and documented, and that all resident and facility needs are met. + Assisting with meal preparation processes – including diets, menus, and sanitation – in accordance with policy. + Utilizing communication systems to ensure information is shared with the appropriate individuals/programs as necessary. Keeping the Manager and/or designee informed of events, including incidents and serious reportable events. + Ensuring all documentation and paperwork is completed. + Attending required meetings and providing in-service training to staff regarding goal implementation, documentation, and other relevant areas. Ensuring that assigned direct care staff complete all necessary training. + Participating in the on-call system for the facility. + Maintaining the home in a clean, safe, and organized manner (according to monthly home inspection checklists); monitoring supplies/equipment/physical plant, and notifying the manager of any needs. + Adhering to all Agency policies and procedures, NYCRR 624, 633, 635, and other applicable regulations. + Performing any other related duties as directed by the supervisor.

Knowledge, Skills, and Abilities:
+ Ability to work independently and inspire others. + Effective communication skills, both verbal and written. + Must be able to provide shift coverage as needed.

Education and Experience:
+ High school diploma or equivalent required. + An Associate's degree in the human services field, with at least one year of experience working with individuals with I/DD preferred.

Physical Requirements/Working Conditions:
+ Must be able to stand, walk, and stoop frequently. + Must be able to push/pull repeatedly. + Ability to lift at least 50 lbs. of dead weight.

Corporate Qualifications/Expectations:
+ Must be at least 18 years of age. + Must meet applicable Vehicle Operator Requirements. (NYS Driver's License Required) + Adhere to all CDS Life Transitions, Inc. policies and procedures. + Comply with the Agency Mission, Vision, Shared Values, and Customer Service Standards. + Attend mandatory education and training modules as scheduled; obtain and maintain required certifications. + Maintain all required certifications/training as mandated by State regulations and CDS policy. + Represent CDS Life Transitions, Inc. professionally in terms of appearance, behavior, communication, and dress. The listed salary range represents the organization's good faith and reasonable estimate of the range of possible compensation at the time of posting.

The offered salary will be determined by:
Applicant qualifications and experience, education, position-specific licensing/training, and departmental budgets.
