Job Title: Licensed Practical Nurse - Ephrata Digestive Health

We are seeking a skilled Licensed Practical Nurse to join our team at WellSpan Health in Ephrata, PA. As a Licensed Practical Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.

Job Summary:

The Licensed Practical Nurse will be responsible for providing nursing care to patients in accordance with established policies and procedures. This includes assessing patient needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure optimal patient outcomes.

Responsibilities:
  • Obtain patient history and gather information to develop a comprehensive understanding of patient needs.
  • Perform specialized procedures, examinations, and tests as ordered and aligned with the Ambulatory Clinical Scope Document.
  • Ensure all patient care areas are adequately stocked with unexpired supplies and medications.
  • Relay information and facilitate communication between the provider and the patient concerning patient care.
  • Evaluate patient's/caller's health status based upon presenting symptoms and follows defined protocols.
  • Tracks, reviews, and monitors the follow-up of diagnostic test results to ensure the physician/APP have verified results and patient has been notified.
  • Provides related education, counseling, ongoing review, and monitoring regarding diagnostic tests under the collaboration of the Physician/APP.
  • Assists in the management of patient care between visits.
  • Maintains patient records to ensure accurate documentation of the patient's medical treatment.
  • Adheres to Health Insurance Portability and Accountability Act (HIPAA) of 1996.
  • Documents and processes prescription requests and refills per Physician/APP order and or policy.
  • Uses Universal Precautions with every patient and visitor encounter.
Requirements:
  • Diploma Program Certificate of completion (degree or diploma) from an accredited LPN program.
  • 1 year Clinical nursing experience.
  • Licensed Practical Nurse Upon Hire Required.
  • Basic Life Support Upon Hire Required.
