Job Title: LPN - PRN (Day Shift)

We are seeking a skilled Licensed Practical Nurse to join our team at Clay Gardens Place. As an LPN, you will play a vital role in providing high-quality nursing care to our residents.

Job Summary:

The LPN will be responsible for providing comprehensive health and psychosocial evaluation, monitoring of the client's condition, health promotion, and prevention coordination of services. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work effectively in a fast-paced environment.

Responsibilities:

  • Provide nursing care in accordance with the client's plan of care
  • Conduct comprehensive health and psychosocial evaluations
  • Monitor the client's condition and provide health promotion and prevention services
  • Coordinate services with other healthcare professionals
  • Develop and implement individualized care plans
  • Document client progress and communicate with healthcare team members

Requirements:

  • Two years LPN experience required
  • Two years' experience in assisted living or in a nursing home
  • Licensed Practical Nurse license in good standing
  • Ability to ensure community follows all federal, state and local laws and regulations as pertains to resident services and care
  • Demonstrate good judgment, problem-solving, and decision-making skills
  • Effective organization, time management, and written and verbal communication skills
  • The ability to handle multiple priorities and delegate assignments
  • A dedication to seniors and their well-being is essential
  • The ability to work nights and weekends
