Role Summary:

We are seeking a Full-time Hospice Registered Nurse Case Manager who is located in or near Warren County, NJ. In this vital role, you will be an integral part of the interdisciplinary team, working under the guidance of the Director of Nursing, attending physician, and the care team to provide comprehensive nursing assessments, planning, and care that prioritizes the comfort and health of patients and their families.

Key Responsibilities:

  • Manage a designated patient caseload as assigned by the Patient Care Coordinator or their representative.
  • Take primary responsibility for assessing, planning, implementing, and evaluating nursing care for patients.
  • Communicate effectively with attending physicians, hospice staff, and other agencies to ensure coordinated care and resource utilization.
  • Maintain ongoing communication with the attending physician regarding patient status and family needs.
  • Collaborate with team members to enhance the patient care plan and gather additional insights.
  • Document patient care accurately and promptly in electronic medical records.
  • Contribute to Interdisciplinary Group meetings with relevant information.
  • Guide primary caregivers and volunteers in providing physical care to patients.
  • Engage in regular meetings with hospice nursing staff to discuss caseload challenges and share professional insights.
  • Participate in patient care conferences to facilitate discussions on caseload issues.
  • Supervise and document the activities of nursing assistants as required.
  • Notify the Patient Care Coordinator of any unusual or concerning patient or family situations.
  • Assist other primary nurses during on-call hours as needed.
  • Provide support during the time of death and the bereavement period.
  • Participate in orientation and training programs for hospice staff.
  • Engage in community programs to enhance understanding of hospice care.
  • Stay informed about agency policies and relevant state and federal regulations.
  • Accept additional responsibilities as needed.

Qualifications:

  • Current RN license in the State of NJ.
  • A minimum of two years of nursing experience, with at least one year in hospice care preferred.
  • Ability to assess and address the needs of patients and families in diverse environments.
  • Competence in nursing practice, with the ability to manage emotional stress of families and respect individual lifestyles.
  • Strong observational skills and ability to produce detailed, accurate reports.
  • Team-oriented mindset, supporting an interdisciplinary approach to patient care.
  • Valid driver's license, good driving record, and reliable transportation.
  • Capacity to work independently and collaboratively in a professional manner that aligns with the organization's mission.
