Nurse Practitioner in Family Health
Role Summary
As a Nurse Practitioner at Apree Health, your primary focus is on fostering meaningful connections with patients to address their comprehensive health needs. You will engage with approximately 10 to 15 patients daily, conducting 30 to 60-minute consultations, all while being supported by a dedicated care team, including health coaches, to provide holistic care that often requires specialist intervention in the conventional healthcare system.

Key Responsibilities
In this role, you will:
- Embody the mission and values of Apree Health
- Deliver evidence-based primary care to a daily patient load, emphasizing preventive health, managing chronic and acute conditions, and providing comprehensive case management
- Collaborate with patients and your care team, including health coaches, to formulate effective biopsychosocial care plans tailored to each patient's unique needs
- Ensure thorough care in line with the Apree health model, including gathering detailed health histories, performing physical assessments, conducting diagnostic tests, and prescribing medications, all facilitated by onsite lab and pharmacy services
- Evaluate patient population needs and work collaboratively to encourage participation in health evaluations and wellness initiatives
- Provide clinical leadership through active participation in team meetings, effective communication, prioritization of key initiatives, and support for process improvements
- Meet all contractual obligations and expectations in partnership with your care team
- Participate in organizational activities and committees to enhance Apree's operational processes and programs
- Achieve targeted health outcomes by improving the health status of the patient population and meeting established clinical benchmarks
Qualifications
Ideal candidates will possess:
- MD, DO, or NP Board Certification with a focus on primary care, family medicine, or internal medicine
- A valid and unrestricted medical or ARNP license along with a valid DEA number
- A minimum of 3-5 years of experience in primary care
- At least 2 years of experience in case management with an emphasis on health prevention and population health
- Experience in team-based care
- Familiarity with EMR systems, preferably NextGen
- Knowledge of coding and documentation practices, including Medicare Advantage and HCC
- Proficiency in Google Workspace
Compensation
Salary range: $115,000 - $138,000 annually, with eligibility for bonuses based on performance.
