Family Nurse Practitioner

We are seeking a highly skilled Family Nurse Practitioner to join our team as a Primary Care Specialist. As a key member of our care team, you will provide evidence-based primary care to patients, focusing on prevention and delivering practical treatment for chronic and acute care needs.
Key Responsibilities- Provide comprehensive care to patients, including collection of comprehensive health history, performing appropriate physical examinations, conducting diagnostic testing, and prescribing medications.
- Partner with patients and the care team to develop effective biopsychosocial patient care plans that meet the needs of each unique patient.
- Assess patient population needs and work with team members to drive participation in annual whole health evaluations, health coaching, and other health promotion activities.
- Provide clinical leadership, including participation in team meetings, clear proactive communication, regular prioritization of key efforts, process improvement, and actively supporting change management.
- MD, DO, or NP Board Certification with primary care, family medicine, or internal medicine background.
- Current, valid and unrestricted medical or ARNP license and valid DEA number.
- Minimum 3-5 years of primary care experience.
- Minimum 2+ years with case management emphasizing health prevention and population health management.
