The Licensed Master of Social Worker is responsible for providing consultation and service to homebound individuals in need of assistance with complex social and emotional problems that impede management of medical regimens.
Key Responsibilities:- Perform comprehensive assessments of patients and caregivers to identify needs and develop appropriate plans of care.
- Establish specific goals and regularly evaluate progress toward these goals.
- Develop person-centered plans of care that promote holistic approaches to optimal medical, emotional, safety, and well-being of patients and caregivers.
- Continually evaluate patient needs and modify care plans as necessary in collaboration with patients and caregivers.
- Master's Degree in Social Work.
- Two years of social work experience in a healthcare setting.
- Licensure as a Social Worker in the State of Connecticut.
