Educational Qualifications: Candidates must be graduates of an accredited Speech-Language Pathology program with a Master's Degree or higher. A valid ASHA certification in Speech-Language Pathology or participation in a Clinical Fellow Year Program is required.

Experience Requirements: A minimum of one year of experience in a long-term care setting is essential.

Licensure and Certification: Applicants must possess a valid NYS license as a Speech-Language Pathologist or be a Clinical Fellow under appropriate supervision from a certified Speech-Language Pathologist. ASHA certification or participation in a Clinical Fellow Year Program is also necessary.

Key Responsibilities:

  • Assess residents to determine their rehabilitation therapy needs.
  • Conduct Speech-Language Pathology evaluations as ordered by physicians.
  • Develop and implement individualized Speech-Language Pathology treatment plans.
  • Adjust treatment strategies based on residents' responses to therapy.
  • Document residents' progress towards their Speech-Language Pathology goals.
  • Facilitate the discharge process for residents when appropriate.
  • Maintain accurate records, including Daily Activity Schedules and progress notes.
  • Ensure compliance with Medicare Part A PPS case management protocols.
  • Adhere to all Federal and State regulations related to rehabilitation therapy.
  • Utilize clinical software applications in accordance with facility policies.
  • Participate in interdisciplinary team meetings for discharge planning.
  • Maintain confidentiality of resident information and facility policies.
  • Report any instances of potential resident abuse or neglect immediately.
  • Contribute to the continuous improvement of care quality.
