Hospice Care Coordinator

We are seeking a highly skilled Hospice Care Coordinator to join our team at Skagit Regional Health. As a key member of our interdisciplinary team, you will be responsible for coordinating patient care and ensuring that patients receive the highest level of quality care in the comfort of their own homes.
**Responsibilities:**
- Coordinate patient care and develop personalized care plans that address the physical, emotional, and spiritual needs of patients and their families.
- Collaborate with other healthcare professionals to ensure seamless coordination of care and optimal outcomes.
- Deliver high-quality nursing care that is respectful, compassionate, and responsive to the needs of patients and their families.
- Maintain accurate and detailed records of patient interactions, including assessments, care plans, and progress notes.
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
**Requirements:**
- Bachelor's degree in Nursing (BSN) or related field.
- Current license to practice as a Registered Nurse in the state of Washington.
- Minimum three years of experience in hospice care or a related field.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively as part of a team.
**Benefits:**
