Job Summary

The Care Coordination Specialist will be responsible for coordinating patient care across multiple settings, including hospitals, clinics, and community-based organizations. This role requires strong communication and interpersonal skills, as well as the ability to navigate complex healthcare systems.

Key Responsibilities

  • Develop and implement care plans that address patient needs and goals
  • Collaborate with healthcare providers and other stakeholders to ensure seamless transitions of care

Requirements

To be successful in this role, you will need:

  • A BSW or BA/BS in Human Services, Health Sciences, or related field
  • Experience in case management, care coordination, or a related field
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Familiarity with electronic health records and other healthcare technologies
