Job Title: LVN Care Coordinator

At Titanium Healthcare, we are seeking a highly skilled and compassionate LVN Care Coordinator to join our team. As a key member of our Enhanced Care Management (ECM) team, you will play a vital role in coordinating care for our members, ensuring they receive the best possible support and services.

Job Summary

The LVN Care Coordinator will work closely with our interdisciplinary team to provide high-quality, effective care management to our ECM members. This includes facilitating access to primary care and behavioral health providers, engaging members in skilled therapeutic interactions, and coordinating care to ensure seamless transitions.

Responsibilities
  • Facilitate access to primary care and behavioral health providers as needed
  • Work effectively with the interdisciplinary team to provide high-quality care management to ECM members
  • Engage members in skilled therapeutic interactions to promote health behaviors and other behavioral health interventions
  • Coordinate care, including resource linkages, with other professionals and organizations in the community
  • Ensure seamless transitions of care that meet IEHP's requirements for post-ER visits and hospitalizations
  • Complete regularly scheduled follow-up visits
  • Provide care coordination through phone calls, emails, messages, updating calendars, scheduling appointments, data entry, and attending meetings
  • Support BH Care Manager, Community Health Worker, and Care Coordinator with delegated tasks
  • Collaborate on member care issues with other ECM care team members
  • Participate in weekly systematic case reviews and ad hoc case reviews
  • Consult with the Clinical Nurse Leader and/or the Behavioral Health Care Manager before taking any clinical action
  • Strategize to engage members in care and conduct motivational interviewing
  • Provide health education to members within scope of practice
  • Initiate a care plan and complete all necessary assessments per healthcare plan guidelines
  • Understand and abide by all departmental and companywide policies and procedures
  • Perform other duties as assigned or required
Requirements
  • High School Diploma/GED
  • Minimum 2 years of experience as an LVN working in a healthcare setting, such as a medical clinic or acute care hospital
  • Experience with an EHR/EPM
  • Valid California driver's license and clean driving record
  • Reliable vehicle and valid automobile insurance
  • Valid, unrestricted state LVN license
Preferred Qualifications
  • Associate degree
  • eClinical Works (ECW) experience a plus
  • Experience working in an internal medicine clinic
  • Care management experience
  • Bilingual in Spanish
  • Valid BLS certification from the American Heart Association
  • Valid CPR Certification
