Job Summary

We are seeking a skilled and compassionate Private Duty Nurse LVN to join our team. As a Private Duty Nurse, you will provide one-on-one care to our patients in the comfort of their own homes, assisting them with transitioning from hospital care to home-based care.

Responsibilities
  • Implementation of physician-prescribed plans of care
  • Patient assessments and coordination of care
  • Administration of prescribed medication, treatments, and therapies
  • Compliant, timely documentation of care provided
  • Health promotion, teaching, and training of family members
Qualifications
  • Active, unencumbered license (RN/LPN/LVN) in the state of practice
  • Current CPR certification from American Red Cross, American Heart Association, or BLS
  • TB skin test (current within last 12 months)
  • Six months prior hands-on nursing experience preferred but not required
