Job Summary

We are seeking a highly motivated and compassionate Behavioral Health Advocate to join our team at Chinatown Service Center. In this role, you will work directly with patients and clinic providers to provide case management services that implement the clinical treatment plan.

Key Responsibilities:

  • Conduct pre-screenings and assessments to develop comprehensive care plans.
  • Coordinate and monitor medical treatments to ensure seamless patient care.
  • Connect patients with community resources to promote positive life outcomes.
  • Provide ongoing follow-up and support to patients and their families.
  • Participate in team meetings and contribute to clinic development.

Requirements:

  • Bachelor's degree in healthcare, social services, human services, or gerontology-related field.
  • 1 year of experience in behavioral health, preferably with 2 years of case management experience.
  • Knowledge of principles and techniques of social, cultural, and intergenerational dynamics of Chinese/Asian immigrants and refugees.
  • Bilingual/bicultural English and Chinese required.
  • Excellent interpersonal, communication, and organizational skills.
  • Commitment to cultural competency and client-centered care.

Salary: $55,000 - $65,000 per year, commensurate with experience.
