Job Summary
We are seeking a highly motivated Community Health Advocate to join our team at South Central Family Health Center. As a key member of our outreach team, you will play a critical role in connecting patients, families, and community members with educational and community resources in the South Los Angeles area.

About the Role
The successful candidate will be responsible for recruiting and retaining patients at various SCFHC Clinic locations, as well as engaging with faith-based organizations, schools, and community-based organizations (CBOs) to develop partnerships and promote our services. You will also be responsible for maintaining accurate records and inventory of outreach materials and equipment.

Key Responsibilities
• Patient recruitment and retention at SCFHC Clinic locations
• Developing partnerships with community organizations
• Maintaining accurate records and inventory of outreach materials and equipment
• Communicating with individuals in a culturally competent and professional manner

Requirements
• High School Diploma or equivalent
• CPR/First Aid Certification
• Knowledge of community resources in the area
• Minimum 1-3 years' experience in a community clinic or FQHC setting
• Current California Driver's License
• Bilingual and bi-literate Spanish/English required
