We are currently seeking a Community Liaison to enhance our outreach efforts and support our patients and their families.

Key Responsibilities:

  • Evaluate the potential of our services to identify target demographics.
  • Engage with healthcare professionals in various settings, including hospitals and assisted living facilities, to present our services and secure patient referrals.
  • Conduct thorough analyses of our marketing strategies and service offerings.
  • Develop and implement a strategic marketing plan with clear priorities and timelines.
  • Facilitate coordination of referrals across all Cedarhurst Senior Living services.
  • Participate in interdisciplinary group meetings as needed.
  • Serve as the primary contact for clinical referral sources and provide initial informational sessions for patients and families.
  • Assist with administrative tasks related to patient documentation and updates.
  • Conduct community outreach and educational initiatives.
  • Support discharge planning processes as required.

Qualifications:

  • A minimum of one year of experience in marketing, nursing, or social work within healthcare settings.
  • Preferred clinical licensure and experience in hospice or home care.
  • Must possess a valid driver's license and have access to a reliable vehicle.
  • Willingness to travel locally and occasionally for overnight stays.
  • Strong verbal and written communication skills.
  • Ability to meet the physical demands of the role, including lifting and moving patients as necessary.
  • Proven ability to work collaboratively and take initiative in a team environment.
