About the Role

We are seeking a compassionate and detail-oriented Patient Care Navigator to join our team at Gateway Community Health Center. As a Patient Care Navigator, you will work closely with patients, providers, and staff to ensure seamless coordination of care and effective communication.


Key Responsibilities
  • Patient Navigation: Coordinate appointments, communicate with patients and providers, and ensure timely completion of referrals, consults, and diagnostic testing.
  • EHR Management: Accurately document patient interactions and maintain up-to-date records in our EHR system.
  • Specialty Provider List Maintenance: Update and maintain our list of specialty providers, including payor sources accepted.

Requirements
  • Education: High school graduate or equivalent required; vocational/technical school diploma or certificate preferred.
  • Certifications: Registered/Certified with National Association for Healthcare Professionals, American Allied Health, or state registration/certification within 3 months of hire.
  • Experience: Two years' experience in medical office assistant or related field preferred.
  • Languages: Bilingual English/Spanish language skills preferred.

Estimated Salary:

$45,000 - $55,000 per year, depending on experience.
