About the Role

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our Home Health team in West Orange, New Jersey.

As a Home Health Nurse, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence, dignity, and comfort.

Responsibilities
  • Assess and evaluate patient needs, developing personalized care plans to address physical, emotional, and social needs.
  • Provide skilled nursing care, including wound care, medication management, and education on self-care techniques.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordinate services.
  • Document patient progress, updating care plans and outcomes as needed.
  • Identify teaching needs and develop strategies to educate patients and families on health management and disease prevention.
  • Monitor quality and appropriateness of services provided, ensuring adherence to agency policies and procedures.
Requirements
  • RN with Bachelor's degree in Nursing, preferred.
  • 1+ year of supervised clinical experience as a registered nurse.
  • Excellent verbal and written communication skills.
  • Strong computer skills.
  • Excellent organizational and interpersonal skills.
  • Valid driver's license and reliable transportation.
