Job Title: Registered Nurse Home Health

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our Home Health team in West Orange, New Jersey.

Job Summary:

We are looking for a dedicated and experienced Registered Nurse to provide high-quality patient care in the comfort of their own homes. As a Registered Nurse Home Health, you will be responsible for assessing patients' needs, developing individualized care plans, and delivering evidence-based care to promote optimal health outcomes.

Responsibilities:
  • Assess patients' physical, emotional, and social needs to develop comprehensive care plans
  • Provide skilled nursing care, including wound care, medication management, and education
  • Collaborate with interdisciplinary teams to coordinate patient care and services
  • Document patient progress and outcomes in a timely and accurate manner
  • Participate in quality improvement initiatives to enhance patient care and services
Requirements:
  • Current RN licensure in New Jersey
  • 1+ year of experience in home health or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong organizational and time management skills
