VNA Health Group is seeking a skilled and compassionate Home Health Nurse to join our team. As a Home Health Nurse, you will provide high-quality patient care to individuals in their homes, promoting independence and well-being.
Responsibilities:- Assess and evaluate patient needs, developing individualized care plans to meet their unique requirements.
- Provide skilled nursing care, including wound care, medication management, and education on disease management and prevention.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Monitor patient progress, updating care plans and outcomes as needed.
- Identify teaching needs and develop strategies to educate patients and families on health management and self-care.
- RN with a Bachelor's degree in Nursing, preferred.
- 1+ year of supervised clinical experience as a registered nurse.
- Excellent verbal and written communication skills.
- Strong computer skills and ability to navigate electronic medical records.
- Excellent organizational and interpersonal skills.
