At New York Cancer & Blood Specialists, we're innovators reshaping modern medicine. As a Breast Health Nurse Navigator/Procedural RN, you'll be part of a rapidly growing organization serving over 50,000 patients annually.

Key Responsibilities:
  • Collaborate with the interdisciplinary team to develop a customized plan of care for each patient, reviewing instructions and scheduling appointments.
  • Facilitate interaction between patients and their physician and referring physician(s) as needed.
  • Navigate patients and families across care settings according to established patient care plans and clinical pathways.
  • Utilize effective communication to ensure patients and caregivers have the necessary information, understanding, and tools to adhere to individualized treatment plans.
Requirements:
  • Bachelor's Degree in Nursing
  • Current New York State Registered Professional Nursing License
  • Excellent communication, interpersonal, and organizational skills
  • Demonstrate consistent professional conduct and meticulous attention to detail
  • Proficiency in Electronic Health Record documentation
  • Minimum of two years experience in an outpatient medical office setting
  • Healthcare or Customer Service Experience
  • Patient Navigation Certification (NYCBS will support team members in achieving and maintaining specialized certification)
