Job Summary

We are seeking a highly skilled and compassionate Social Services Director to join our team at Carmel Hills Wellness & Rehabilitation. As a key member of our interdisciplinary team, you will be responsible for providing exceptional social services to our residents, ensuring their psychosocial needs are met, and promoting their overall well-being.

Key Responsibilities
  • Conduct thorough social histories and psychosocial assessments to identify residents' needs and develop personalized care plans.
  • Maintain accurate and timely documentation, adhering to federal, state, and local regulations, as well as our company's policies and procedures.
  • Support residents' rights to self-determination, documenting their preferences and involving them in the decision-making process.
  • Participate in the development of interdisciplinary care plans, ensuring residents' psychosocial needs are addressed.
  • Provide therapeutic interventions to assist residents in coping with their transition and adjustment to our facility.
  • Coordinate discharge planning, making referrals for home care services as needed.
  • Collaborate with the interdisciplinary team to promote and protect residents' rights and well-being.
Requirements
  • Certified, licensed, or registered in the state of practice, required.
  • Bachelor's Degree in Social Work or a related field, including Sociology, Special Education, Rehabilitation Counseling, and Psychology, required.
  • One year of supervised social work experience in a healthcare setting, working directly with geriatric individuals, required.
  • Minimum of 2 years of nursing experience in a Skilled Nursing Facility, preferred.
