JOB SUMMARY:

The Practical Nursing Specialist is accountable for making informed decisions grounded in their educational background and nursing expertise, while delivering nursing services as outlined in the patient care plan. This role involves reporting any issues or potential concerns to the RN/Case Manager.


DUTIES AND RESPONSIBILITIES:

  • Engage in the nursing process by contributing to the client's care plan.
  • Execute the care plan as prescribed.
  • Assess the effectiveness of the care plan and suggest modifications as necessary.
  • Document all relevant observations and treatments in accordance with agency protocols.
  • Prepare clinical progress notes and ensure proper documentation of services in line with agency policies.
  • Ensure the emotional and physical comfort and safety of clients.
  • Perform nursing tasks for which proper training has been received.
  • Alert the appropriate personnel regarding any unusual symptoms or changes in client conditions.
  • Participate in case discussions, team meetings, and performance enhancement activities as assigned.
  • Provide proof of current licensure by the state and necessary documentation to verify education and work experience.
  • Engage in in-service training programs.
  • Practice safely and competently within the defined job description.
  • Adhere to all state regulatory requirements.
  • Comply with all agency policies and procedures, including HIPAA Privacy Rules.
  • Maintain confidentiality regarding patient and business information.

QUALIFICATIONS:

Must be a graduate of an accredited nursing program and currently licensed in the state. A minimum of one year of recent experience in an accredited hospital or nursing facility within the last two years is required.
