The Licensed Practical Nurse - Patient Care Specialist is responsible for delivering high-quality patient care services as ordered on the patient plan of care. This role requires a strong foundation in nursing principles and practices, with a focus on providing compassionate and individualized care to patients. The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a team environment.

Key Responsibilities:
  1. Participate in the nursing process by providing input to the patient's plan of care.
  2. Implement the care plan as written, ensuring that all necessary interventions are provided in a timely and effective manner.
  3. Evaluate the effectiveness of the care plan, making recommendations for revisions as appropriate.
  4. Record all pertinent observations and treatments as per agency policies, maintaining accurate and detailed clinical progress notes.
  5. Provide for the emotional and physical comfort and safety of the patient, promoting a positive and supportive care environment.
  6. Perform nursing procedures for which they have been trained, adhering to established protocols and guidelines.
  7. Notify appropriate personnel about unusual symptoms and/or changes in the patient's condition, ensuring prompt and effective intervention.
  8. Participate in case conferences, team meetings, and performance improvement activities as assigned, contributing to the development and implementation of quality improvement initiatives.
  9. Comply with all state regulatory acts and agency policies, including HIPAA Privacy Rules, maintaining the highest standards of confidentiality and professionalism.
Requirements:
  • Must be a graduate of an approved school of nursing and currently licensed in the state.
  • Must have one year of current experience in an accredited hospital or accredited nursing home within two years previous to employment.
