Job Description\
As a Case Manager at Urban Pathways, you will play a crucial role in delivering clinical and case management services to assigned clients. Your primary responsibilities will include: \
• Providing crisis intervention, advocacy, and assistance in problem-solving. \
• Assessing client needs and developing service plans to achieve self-sufficiency. \
• Conducting field work, including home visits, escorting clients to appointments, and outreach. \
• Maintaining accurate records and reports as required by regulatory and funding agency policies. \
• Collaborating with community services and resources to ensure holistic support.\
\
Requirements\
We seek a highly motivated individual with a bachelor's degree in social work or a related field. A minimum of one year of experience in clinical or case management settings is essential. Strong computer skills, excellent communication, and interpersonal abilities are also required. Bilingual English or Spanish proficiency is a plus.\
\
Salary and Benefits\
