Job SummaryWe're looking for a skilled Physical Therapist to join our team. As a Physical Therapist at JAG, you'll have the opportunity to work with a diverse patient population, providing expert care and guidance to help them achieve their goals. Your strong clinical skills, excellent communication, and ability to work effectively in a team environment will make you a valuable asset to our team.
Key Responsibilities- Collect and record patient care data, ensuring accuracy and compliance with regulatory agencies and third-party payers.
- Develop and implement treatment plans, incorporating evidence-based practices and patient-centered care.
- Collaborate with interdisciplinary teams, patients, and support systems to ensure seamless care coordination.
- Provide education and training to patients and their support systems, promoting independence and self-management.
- Conduct initial evaluations, daily notes, reassessments, and progress reports, ensuring timely and effective communication with the team.
- Bachelor's or Master's Degree in Physical Therapy, with a Doctorate degree preferred.
- Licensed Physical Therapist in the state of practice.
- Completed state-regulated CEU requirements.
- Valid CPR license.
- Experience with EMR software, with a strong understanding of reimbursement systems and their impact on patient care.
