Job Title: Retail Health Coordinator

Job Summary:

This role requires a highly motivated and organized individual to work in our Wellness room, coordinating patient care and communicating with plan and retail health identified members telephonically. You'll work closely with our clinical operation leader and local leader to ensure seamless care delivery.

Key Responsibilities:
  • Coordinate patient flow and schedule appointments
  • Interact with customers, patients, and caregivers telephonically and through technology-based interactions
  • Reach out to language line/interpreter services when needed
  • Schedule and track patient referrals and diagnostic tests
  • Document patient information and care plans
  • Provide superior customer service and communicate effectively with internal and external stakeholders
Requirements:
  • High School Diploma or equivalent required; Associate's Degree preferred
  • Minimum 3-5 years of healthcare, customer service, patient relations, and/or hospitality concierge experience
  • Natural passion for engaging and connecting members telephonically and communicating health plan identified gaps
  • Strong computer, data entry, and typing skills; ability to work between multiple technology platforms
  • Proficiency in Microsoft Office applications, including Outlook, Word, and Excel
  • Excellent time management and organizational skills
  • Ability to multitask and prioritize while maintaining a pleasant and calm demeanor
Preferred Qualifications:
  • Medical Assistant certification or experience in a doctor's office or coordination of care
  • Experience in a call center or telephonic care management setting
  • Certified Pharmacy Technician (CPhT) or other medical/customer service certification
  • Prior experience handling sensitive information confidentially, including patient health information (PHI) protected under HIPAA
