Job Description

Job Summary:

The Senior Services Case Manager will play a critical role in the well-being of aging individuals and their families. This position will serve as the primary contact for people in the community seeking information and referral, care coordination, and case management for elders in the community aging in place, and for Jewish elders living in supportive, assisted living, or other non-independent housing.

Key Responsibilities:

  • Care Planning and Coordination: Develop and implement care plans for senior clients, including assessment of physical, emotional, psychological, and social needs. Provide health care navigation services, including coordination with out-of-area adults, elder law attorneys, and facility social workers as needed.
  • Client Education and Support: Provide education and counseling to clients, families, and caregivers regarding health care needs and available resources. Develop and implement strategies to resolve care-related issues.
  • Referral and Resource Navigation: Assist with referrals to appropriate community services and resources. Plan for acute hospital discharge needs, including arrangements for follow-up care and home health services, in collaboration with hospital social workers.
  • Advocacy and Crisis Intervention: Advocate for clients to ensure their physical and emotional needs are met. Provide crisis intervention when needed.
  • Community Outreach and Engagement: Coordinate JAMP (Jewish Aging Mastery Program), including but not limited to coordination of program speakers, arranging for space, advertising, registration, and oversight during programs themselves. Connect and act as a liaison between JFS and local synagogue staff, including but not limited to identification of elders in need of supportive services, and community needs.
  • Program Development and Implementation: Coordinate and lead community-based support groups related to topics pertinent to the Jewish Community, including but not limited to grief and loss groups, caregiver support, living with a partner with Dementia, etc. Provide onsite consultations and personal needs assessments to community elders during food pantry hours and for other JFS events.
  • Reporting and Data Collection: Maintain monthly senior newsletter providing news, events, and senior-related information to the local community. Oversee financial assistance program (Back on Track, or BOT) to seniors in need of specific financial assistance, as well as to other clients as identified by JFS program staff. Data collection related to services provided and assistance in reporting to grant organization as needed.

Requirements:

  • Knowledge and Skills: Knowledge of the aging process and its physical, psychological, and social implications. Knowledge of the health care system and available resources for the elderly. Understanding of the roles and responsibilities of a geriatric case manager and health care navigator.
  • Assessment and Planning: Ability to assess patient needs and develop comprehensive care plans. Ability to develop and implement strategies to facilitate patient care objectives.
  • Communication and Collaboration: Ability to communicate effectively with clients, family members, healthcare providers, and other stakeholders. Ability to coordinate with other professional staff to ensure timely, efficient, and cost-effective care.
  • Problem-Solving and Critical Thinking: Strong problem-solving skills. Knowledge of applicable laws, regulations, and standards of practice related to senior care.
  • Technology and Data Management: Proficiency in computer systems, applications, and technology related to case management.
  • Cultural Competence: Knowledge of the Greater Harrisburg Jewish Community and its customs and culture.
