Job Summary:

This role is responsible for providing clinical support and coordination of patient services in a high-volume, fast-paced call center environment. The Nurse Navigator will work closely with patients, caregivers, and healthcare providers to ensure seamless coordination of care and access to necessary services.

Key Responsibilities:

  • Provide clinical triage and assessment of patient needs, prioritizing treatments according to urgency and collaborating with multidisciplinary teams to coordinate care.
  • Assist patients and caregivers with accessing clinical and supportive care services, including scheduling appointments and coordinating with healthcare providers.
  • Identify patient needs and provide education, screening, support, referrals, and coordination of care as necessary.
  • Work effectively as a team member, offering support and ideas to enable the group to reach its goals.
  • Perform other duties as assigned or required.

Requirements:

  • High School / GED
  • 1 year of Nursing in the specialty
  • Act 34 Criminal Background Clearance Certificate
  • Act 33 Child Abuse Clearance Certificate
  • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
  • Current State of PA RN licensure OR Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC).
  • CPR certification

Preferred Qualifications:

  • Bachelor's Degree in Nursing
  • 1 year in a Emergency Room/ICU role
  • 1 year with adult medicine/surgery
  • 1 year in call center or tele-health setting
  • Certification in area of expertise
  • Valid PA driver's license may be required
  • Older Adults Protective Services Act (OAPSA) clearance may be required depending on location

$30.82
