Job Summary

We are seeking a skilled Care Coordinator to join our team at John Muir Health. As a key member of our interdisciplinary health team, you will play a vital role in ensuring seamless transitions for our patients from hospitalization to community care.

Key Responsibilities
  • Assess patient needs and develop personalized care plans to meet their health care requirements.
  • Collaborate with healthcare professionals to coordinate care, manage transitions, and ensure continuity of services.
  • Provide education and support to patients and their families to promote self-management and independence.
  • Monitor patient progress, identify potential issues, and implement interventions to prevent complications.
  • Stay up-to-date with relevant healthcare policies, regulations, and best practices to ensure compliance and quality care.
Requirements
  • Bachelor's degree in Nursing from an accredited school.
  • Minimum 3 years of experience in medical-surgical or critical care nursing, with a focus on care coordination and case management.
  • Strong knowledge of Medicare and Medi-Cal guidelines, as well as commercial coverage plans and reimbursement models.
  • Ability to work effectively in a fast-paced environment, prioritize tasks, and manage time efficiently.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with diverse stakeholders.
Preferred Qualifications
  • Accredited Case Manager certification (ACMA) or equivalent experience.
  • Knowledge of geriatrics, chronic disease states, and palliative care principles.
  • Experience with electronic health records (EHRs) and other healthcare software systems.
Work Environment

This is a per diem, day shift position with no waiver. The work shift will contain the word 'exempt' on it.
