Job Title: Licensed Practical Nurse - Charge Nurse

We are seeking a compassionate and experienced LVN Charge Nurse to join our team at Touchstone Communities. As a Charge Nurse, you will be responsible for providing high-quality patient care and leading a team of healthcare professionals.

Responsibilities:
  • Provide direct patient care and develop individualized care plans
  • Supervise and mentor a team of LVNs and LPNs
  • Collaborate with other healthcare professionals to ensure seamless care transitions
  • Monitor and maintain accurate patient records
Requirements:
  • Current and valid Texas LVN nursing license or a current and valid LVN/LPN license from a Compact Party State
  • Experience in post-acute care and leadership roles
  • Excellent communication and interpersonal skills
