Job Title: Occupational Therapist Home Health

We are seeking a skilled Occupational Therapist to join our team at Alternate Solutions Health Network LLC. As a Home Health Occupational Therapist, you will play a vital role in providing high-quality care to patients in their homes.

About the Role:
  • Provide skilled occupational therapy services to patients in their homes, as ordered by the attending physician.
  • Develop and implement individualized plans of care to meet the unique needs of each patient.
  • Conduct thorough evaluations to assess patient's level of functioning and identify areas for improvement.
  • Collaborate with patients, families, and healthcare teams to ensure comprehensive care and optimal outcomes.
  • Document patient progress and maintain accurate records, adhering to agency protocols and regulatory requirements.
Requirements:
  • Current licensure as an Occupational Therapist in the state of employment.
  • Minimum one year of experience as an OT in an acute care setting, with home care experience preferred.
  • Valid driver's license and auto insurance.
  • Ability to meet the physical demands of the job.
