We are seeking a skilled RN Case Manager to join our team at CHI St Vincent Infirmary. As a Case Manager Nurse, you will work collaboratively with physicians, registered nurses, and other healthcare professionals to develop and implement effective care plans for patients.
Key Responsibilities- Conduct thorough assessments of patient needs and develop personalized care plans to ensure optimal outcomes.
- Participate in multidisciplinary rounds and length of stay meetings to discuss patient care and progress.
- Collaborate with patients, families, and healthcare teams to identify and address clinical, spiritual, financial, and educational barriers to care.
- Develop and implement discharge plans to meet patient needs, reduce length of stay, and prevent readmissions.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
- Unencumbered license to practice nursing in the State of Arkansas.
- 3 years of acute care nursing experience required.
- BSN preferred, or Associate Degree with CCM or ACM Certification within 2 years of hire date.
- Previous case management, utilization management, or discharge planning experience strongly preferred.
CHI St Vincent Infirmary offers a competitive salary, excellent benefits, and opportunities for professional growth and development. We also offer a $15,000 sign-on bonus to eligible candidates.
