Job Summary

The Case Management Manager Full Time Days position is responsible for coordinating the daily activities of the case management team, ensuring the clinical plan of care conforms to evidence-based practice and regulatory requirements.

Key Responsibilities
  • Coordinate clinical care, ensuring medical necessity, appropriateness of care, and resource utilization align with evidence-based practice and external requirements.
  • Identify and report variances in medical care provided, over/under utilization of resources, and communicate through clear, concise documentation in CM software.
  • Collaborate with physicians, nurses, ancillary staff, payors, patients, and families to achieve optimal clinical outcomes.
  • Remain current with relevant clinical and case management practices.
  • Assist the Director with daily operational oversight and supervision of Departmental staff.
Requirements
  • 2 years of acute hospital case management or healthcare experience.
  • Graduate of an accredited school of nursing.
  • Active RN license for state(s) covered.
  • Preferred: 5 years of acute hospital case management leadership experience, accredited Case Manager (ACM) certification.
