Position Overview:
The Organ Transplant Nursing Coordinator is responsible for overseeing, directing, and delivering specialized nursing care, focusing primarily on individuals with end-stage organ disease who are candidates for solid organ transplantation. This role ensures continuity and coordination of patient care while conducting educational initiatives for both patients and healthcare professionals.
Key Responsibilities:
1. **Transplant Referral:**
- Identify suitable candidates upon referral and engage with relevant staff.
- Review medical records to determine evaluation needs and conduct phone evaluations.
- Collaborate with the Financial Coordinator to secure insurance authorizations.
2. **Transplant Evaluation:**
- Coordinate the evaluation process with transplant team members, adhering to established testing protocols.
- Advocate for patients based on insights gained during discussions with patients and their families.
- Participate in the Patient Selection Committee.
- Provide comprehensive education on transplant evaluations, OPTN regulations, organ allocation, and the entire transplant process, including risks, benefits, and post-operative care.
- Educate on the utilization of expanded and high-risk donors.
- Track testing to ensure all necessary consultations and evaluations are completed, reviewing findings as needed.
- Comply with OPTN/UNOS policies and maintain communication with patients and referring physicians regarding patient status and transplant team involvement.
Minimum Qualifications:
1. Current, unencumbered licensure as a Registered Professional Nurse with the appropriate state board.
2. Certification in Basic Life Support must be obtained within 30 days of hire.
3. A minimum of three years of clinical nursing experience.
Preferred Qualifications:
1. Bachelor’s degree in Nursing.
