Transplant Coordinator

We are seeking a highly skilled and compassionate Transplant Coordinator to join our team at WVU Medicine. As a key member of our transplant team, you will play a critical role in ensuring the smooth coordination of patient care, from initial evaluation to post-transplant follow-up.
Key Responsibilities- Coordinate transplant evaluations, including patient assessments, testing, and education
- Work closely with multidisciplinary teams to develop and implement individualized care plans
- Provide education and support to patients and their families throughout the transplant process
- Collaborate with healthcare providers to ensure seamless communication and coordination of care
- Monitor patient progress and adjust care plans as needed
- Document all interactions and findings in the electronic medical record
- Current licensure as a Registered Professional Nurse in the state of West Virginia or ability to obtain
- Three years of nursing experience, preferably in a transplant or critical care setting
- Basic Life Support certification
- Ability to work independently and as part of a team
- Strong communication and interpersonal skills
- Bachelor's degree in Nursing
- Experience with transplant nursing practice and OPTN/UNOS policies
The Transplant Coordinator will work in a fast-paced, dynamic environment, collaborating with a multidisciplinary team to provide exceptional patient care. The ideal candidate will be able to work effectively in a team setting, communicate clearly and effectively, and prioritize tasks to meet deadlines.
